An AI agent POSTs its actions (agent_id, action_type, tool_name, input/output summaries, metadata) to the /v1/actions API with a Bearer API key; the server appends each record to a hash-chained, append-only ledger: record N stores SHA-256 of record N-1's hash plus its own payload. Any later edit breaks every subsequent link, so the trail is tamper-evident. The wedge is independent post-incident PROOF of what an AI agent did, not agent security.
